I the undersigned, understand and acknowledge that my/minor named above participation in the activities provided at J&K Play Studio, carries inherent risk, which include but are not limited to the following:

1) Physical injuries

Injuries could result from falling/tripping
Collisions with other participants/equipment
Muscular or Skeletal injuries

2) Environmental Hazards:

Allergic reactions to materials or substances present in J&K Play Studio
Risks associated with exposure to infectious disease to include but not limited to colds, flu, and COVID-19

I voluntarily choose to participate in these activities and accept all risks noted or not with my/minors participation.

I am aware that while the staff at J&K Play Studio strive to maintain a safe environment that cannot eliminate risks.

I will not hold J&K Play Studio, employees, or anyone associated liable
